1963. With a few months left on his sentence, thirty-eight-year-old convicted criminal Randle Patrick McMurphy – "Mac" – serving time for several assaults and statutory rape, has just been transferred from a work camp associated with the Eastern Oregon Correctional Institution in Pendleton to a psychiatric hospital. Mac manages to use the pretence of being "crazy" – with aggressive and smart-mouthed behaviour and anti-authoritarian outbursts – to his advantage, so that he does not have to work at all. He is in the hospital because the authorities in Pendleton want him to undergo a psychiatric evaluation to prove that he is not crazy, believing it is all an act to avoid work. He believes this stay will free him from any work until the end of his sentence. He is placed in a ward with a group of men who have varying degrees of clarity of consciousness and control over their mental faculties. He continues to behave as he always has to get what he wants, using the other patients either as accomplices or as a means of his own amusement. To his list of goals he adds the desire to irritate in every way the despotic head nurse of the ward, Mrs. Mildred Ratched, whose obvious desire is to break the spirit of every man under her care. In his struggle with Nurse Ratched, Mac ultimately tries to help the men gain a voice of their own in the hospital, and some of them – for their final return to the outside world.
